• Résumé or curriculum vitae
• Copy of licensure(s), if licensed.
• Copy of certificate(s) of trainings/seminars attended
• Or other documentation of your experience relavant to the treatment and/or evaluation of sex offenders
• Completed Application for Placement on the Provider List
